You have the option to submit this form to NAU Country Insurance by using an electronic signature. By electing to electronically sign, you are agreeing that you have read the disclosures displayed within this document and understand that your electronic signature is binding.

What is an eSignature?

A process by which an applicant, guardian, agent, or substantial beneficial interest (SBI) signs forms, transactions, or documents and creates legally enforceable agreements.

What are the benefits of electronic signatures?

It eliminates the need for you to print, sign, and send documents to NAU Country Insurance.

How does the electronic signature process work?

Once you have reviewed the document/information, you will sign the document by accepting the disclosures and agreeing this signature is binding. The date and time that the document is signed is tracked within the system along with an electronic version of the document.

Can I obtain a copy of the documents that I electronically sign for my personal records?

Yes. After the document is signed, an electronic copy of the document can be downloaded, saved, and printed.

What if I do not want to sign electronically?

If you do not wish to sign this document electronically, you may print, complete, sign, and submit a PDF version of the document via email, fax, or mail.
